For External Collaborators

The Health Professionals Follow-Up Study has a proud history of successful collaborations with investigators outside of the Harvard T.H. Chan School of Public Health and Harvard medical community.

We welcome new collaborations and strive to make establishing them as simple and transparent as possible. Limits are not placed on scientific questions or methods, and there is no requirement for co-authorship.

 Please follow the steps below for any external collaboration requests:

  1. Investigators interested in collaborations are invited to fill out this simple form that asks about the details of the collaboration.
  2. After submitting an external collaboration request, please email Dr. Lorelei Mucci (lmucci@hsph.harvard.edu) for guidance on additional requirements.
  3. Requests to use HPFS data must be presented at at an upcoming HPFS meeting. Please fill out this HPFS meeting form to get onto an HPFS meeting agenda.
  4. Email Kathleen Florentine (kflorentine@hsph.harvard.edu) to get onto the HPFS email list. If you do not get on the HPFS list serv, you will not receive the meeting agenda and zoom information.

**If you sign up for a HPFS meeting date, you can assume that you are on the agenda for that date unless otherwise notified**

The questionnaires on this website are protected by copyright. All rights are reserved. The content and design of these questionnaires may not be used in any way for commercial purposes without permission. In general, use of the questions for scientific research is allowed. Please cite the webpage with the questionnaire in the published research.

If you intend to use the food frequency questionnaire to collect dietary data that will be processed by our group, please do not use copies from this website; you must obtain forms that can be scanned. For more information, please visit the Harvard T.H.
