Even in the top epidemiology journals\, fewer than half of studies using explicitly causal methods (g-methods or TMLE) state an explicitly causal question and fewer than half mention consistency and positivity. There is no reason that all such articles should not always include a causal question\, clearly-stated causal assumptions and supporting arguments for those assumptions. When a causal claim (however tentative or weak) is made in the absence of these\, I refer to this as “dark causal inference”: causal inference that is implied but not observable (in analogy to dark matter). There are many reasons dark causal inference is overwhelmingly abundant in epidemiology of which I will explore two: 1) our continuing misuse of the word “association” (and similar non-causal language) and 2) failing to teach students how to reason about causal inference when causal assumptions are not satisfied.     \n\n\n\nBio: Jeremy Labrecque is assistant professor of epidemiology and leader of the Causal Inference Group at Erasmus MC in Rotterdam\, the Netherlands. Much of the evidence is based on epidemiological studies that have a single baseline measure. The Harvard cohorts have repeated measures\, up to every two years\, over four decades of follow-up. This unique resource has enabled a deep look at various topics\, including the potential for reverse causation\, refinement of dose-response relationships\, the influence of maintaining an active lifestyle over decades\, and the relative influence of moderate versus vigorous activity. Recent insights into these topics will be summarized.  \n\n\n\nBio: Dr. Ed Giovannucci is a Professor of Epidemiology and Nutrition at the Harvard TH Chan School of Public Health\, as well as an American Cancer Society Clinical Researcher Professor. Dr. Giovannucci graduated from Harvard University in 1980\, received an MD from University of Pittsburgh in 1984\, and received training in anatomic pathology before attaining a doctorate in epidemiology at HSPH in 1992. Over the past four decades\, he has conducted studies of diet\, physical activity\, body weight and metabolic health in relation to cancer and other chronic diseases. This joint appointment reflects her interest both in empirical work describing the world as it is and normative scholarship about how the world ought to be. She primarily studies US health care through an organizational lens\, applying theoretical frameworks from business ethics and political philosophy to managerial and policy dilemmas. Epidemiologists are taught to rely on science\, reason\, and facts in their work and yet\, it is likely that we use intuition all the time. We use it when we make decisions about interpreting the results of studies\, about the impact of sources of bias\, in the decisions we make in our own research\, in our teaching and in our communication with other scientists and the general public. Yet it isn’t clear when this approach is useful and when it causes harm. This talk will focus on the role of intuition in epidemiology with examples of cases where we rely on it too much and cases where we don’t use it enough. \n\n\n\nBio: Matthew Fox\, DSc\, MPH\, is a Professor in the Departments of Epidemiology and Global Health at Boston University. Dr. Fox joined Boston University in 1999. His research interests include treatment outcomes in HIV-treatment programs\, infectious disease epidemiology (with specific interests in HIV and pneumonia)\, and epidemiologic methods. Dr. Fox works on ways to improve retention in HIV-care programs in South Africa from the time of testing HIV-positive through long-term treatment. As part of this work\, he is involved in analyses to assess the impact of changes in South Africa’s National Treatment Guidelines for HIV. Dr. Fox also does research on quantitative bias analysis and co-authored a book on these methods\, Applying Quantitative Bias Analysis to Epidemiologic Data. He is also the host of a public health journal club podcast called Free Associations designed to help people stay current in the public health literature and think critically about the quality of research studies and a podcast on Epidemiologic Methods called SERious Epi. Participants should emerge with a better understanding of implementation science and how it can be useful to epidemiologists.  \n\n\n\nBio: Karen Emmons is a Professor of Social and Behavioral Science at the Harvard T.H. Chan School of Public Health. She has expertise in cancer disparities and in implementation science.  Her work focuses on community-engaged approaches to implementation of cancer prevention and early detection interventions in settings that serve low income and disadvantaged communities. She has more than 375 peer-reviewed publications and has served as PI for multiple NIH-funded research projects and centers.  She also serves as Faculty Director of the Community Engagement Program for Harvard’s Clinical Translational Science Award\, Harvard Catalyst\, which focuses on advancing evidence-based strategies to improve community health and creating bi-directional relationships between community partners and researchers. She serves as the MPI of Harvard Chan’s T32 training grant in cancer prevention and control and has received multiple mentoring awards.  Her research has also been recognized by honors including the Morse Distinguished Researcher Award\, Outstanding Senior Cancer Scientist Award from the Society of Behavioral Medicine\, and Outstanding Scientist Award from the Society of Behavioral Medicine. Yet dementia develops over a long prodromal period\, during which subtle vascular cognitive changes may emerge years before clinical onset of cardiovascular disease\, long before dementia is diagnosed. Could detecting the early subclinical cognitive impairment signal cardiovascular vulnerability and open a window for earlier prevention of both cardiovascular and cognitive outcomes? In this talk\, Dr. Zhang will present recent work examining the extent to which global and domain-specific cognitive function\, assessed in mid-to-late adulthood\, predicts incident cardiovascular disease beyond traditional risk factors across high-risk clinical cohorts and community-based general populations. The findings suggest that a brief processing-speed test may capture vascular vulnerability not reflected in conventional risk assessment and could help refine cardiovascular risk stratification in older adults.   \n\n\n\nBio: Wenxin Zhang\, MBBS\, PhD\, is a postdoctoral research fellow in the Department of Epidemiology at Harvard T.H. Chan School of Public Health. Dr. Zhang’s research lies at the intersection of cardiovascular and aging epidemiology\, with the goal of advancing the prevention of cardiovascular disease and dementia and promoting healthy aging. Her research focuses on gender-based drivers of HIV vulnerability among women and children\, and evaluates interventions designed to address these inequities. Currently\, Dr. Kuchukhidze is a Postdoctoral Research Fellow at the Harvard T.H. Chan School of Public Health\, where her work leverages quasi-experimental methods to evaluate the impact of HIV pre-exposure prophylaxis on HIV acquisition risk among pregnant and breastfeeding women in South Africa. When conducting research for clinical and regulatory purposes\, the causal estimand typically involves the causal effect of an intervention. Three frameworks have been proposed to describe the causal estimand in randomized trials and observational studies: PICO\, the target trial framework\, and the E9 (R1) Addendum of the International Council for Harmonisation (ICH). This talks provides a side-by-side comparison of these three estimand frameworks for causal inference\, translate across their terminologies\, and highlight their commonalities and peculiarities.
